Review & Approval
Stage 4 is the stage gate. The readiness board computes, live, whether this package can be approved; the governance rail runs the human workflow: assign reviewers, submit, approve or request changes; and the evidence panel shows what the approval rests on. Approval locks the package immutably and captures the baseline that the Operate stage will watch.
